E-mail communications are not secure, and you should not send personally identifiable or other confidential information by e-mail. When contacting us, preferably by mail, please include the following information so that we can respond more quickly:

• Your name (first and last as it appears on the account)
• Daytime telephone number
• Evening telephone number
• Best time to call
• Product or service (i.e., credit disability insurance, accidental death, warranty, etc.)
• Benefit activation number (if known)
• Certificate/policy number
• Account/loan number
• Creditor/bank/loan company (if applicable)
• Retailer or utility (if applicable)
• Description of problem or concern
